At Downsview Clinic Cardiology and Ultrasound we are committed to protecting the confidentiality of our patients.

All employees of Downsview Clinic Cardiology and Ultrasound are accountable for protecting the confidentiality and security of all personal information under their control.

The purpose for collecting personal information is to provide you with the appropriate service and submit this service to The Ministry of Health.

Explanation of collecting personal information will be given before consent. Consent can be written, verbal or implied.

We will limit the collection of your personal information to details that are necessary for the purposes identified above.

Limiting Use, Disclosure and Retention

Information will be used or disclosed for the purpose for which it was collected only, unless required or permitted by law. Personal information of our patients is retained for a time period required by law after which it is appropriately and securely destroyed.

Accuracy

Personal information will be kept accurate, complete and up to date. You can assist us in ensuring your records are current by providing us with any changes in your personal information (such as your new O.H.I.P. version code). You have the right to challenge the accuracy of your personal information and amend it as necessary.

Safeguards

Our systems and procedures are designed to prevent loss, misuse, unauthorized access, alteration or destruction of personal information. Only authorized employees and service providers of Downsview Clinic Cardiology and Ultrasound have access to your personal information and are provided with training regarding privacy. All security policies apply to the contracts and agreements undertaken with external suppliers and service providers. Electronic data transmissions are not 100 % secure. Therefore, we cannot guarantee the security of the information you transmit to us electronically. We do not disclose any information regarding a patient electronically – this is done in person, upon request.
